Adult Supported Living

Our Adult Supported Living services are individually tailored to provide person centred care. We will work with the service user and their family to create a bespoke support package guided by the principles of person-centred planning. We can bring structure, and help people to learn the essentials of daily life such as self-care, laundry, cooking and budgeting. The support plan will take into account each individual’s ability, wishes, preferences and aspirations. The aim of the support and accommodation is to assist each individual to increase their confidence and independent living skills (social capital) to a level where they are able to move on and live independently.

 
Service user profile:
  • Male or female
  • Aged 18+
  • On the Autistic Spectrum
  • Those with a mental health diagnosis and complex challenging behaviour
  • Forensic History stepping down from Forensic services
  • Learning Disabilities

 

Key Features of Service:
  • Our team will work with the service users, family and MDT to source a suitable property
  • Property is sourced through private landlords and Housing Associations
  • Tenancy in service users name
  • All properties will have appropriate safety checks in line with the Land Lords Act
  • Inclusive of 24 hours on call support

 

Structured programme of support
  • Medication Management
  • Meal preparation
  • Budgeting skills
  • Management of any health issues, e.g. support with registration with GP, dentist and accessing primary care services
  • Participation in education, training and employment
  • Community Involvement to reduce social isolation
  • Support to Improve self-confidence and social/communication skills
